If you are wondering “What is PTSD?” you are not alone. The Congress requested to know and sponsored a nationwide research study to collect data from the DoD/VA to determine the cause of post-traumatic stress (PTSD) and the different treatment methods which heal PTSD best and are most cost-effective. Mostly traditional treatment methods used to heal PTSD were selected for the study. Commonly known is the President’s order for the DoD and VA to select their best ten innovative pilot programs created in the attempt to heal PTSD. Long-standing recommendations of The World Health Organization (WHO) how to heal PTSD have not been considered.

Since the 1980s the question “What is PTSD?” has puzzled the research community. Then, the American Psychiatric Association classified post-traumatic stress (PTSD) as a mental disorder and opened the doors for serious research as to its causes. Depending on the orientation of the particular research team different kinds of belief systems emerged. As a result, different kinds of treatment approaches to healing PTSD were innovated. The advance in biomolecular medicine allowed the increased use of medication to reduce symptoms and the emotional trauma.

Then, the VA was faced with the responsibility to evaluate the most appropriate psychotherapies and chose CBT (cognitive behavioral therapy) as a short-term treatment modality to heal PTSD. It was expected that symptoms of PTSD would be healed, once patients learned to confront their fears and terrors cognitively. The success rate of 50% seemed significant enough to consider CBT treatment a success. Gains achieved did not always last over time. Veterans would return for further PTSD healing or explore different ways to deal with their inner monsters. However, by necessity, too many veterans have been returning for decades for regular brief appointments to secure their prescription medication, in particular for the so-called secondary symptoms of PTSD, e.g. depression, insomnia. Not all patients react well to medication. Often serious side effects have been experienced.

Once Exposure or Virtual Reality therapy were added to CBT and then, EMDR, for faster results in healing PTSD, it has been puzzling to researchers that during the initial therapy sessions, veterans seemed to have healed their PTSD successfully, yet, would report during their follow-up appointment that they felt bad or even worse than before. Or patients would insist how much better they actually felt; yet, tests failed to confirm their convictions.

The answer to this phenomenon can be found in the large global data base of the World Health Organization (WHO), and in quite a few text books which I remember well from my own professional training. The World Health Organization strongly advises against having patients relive, or recount the life-threatening trauma in an effort to heal PTSD. Independent research in Israel, where the exposure therapy had originated, showed the same result: Patients who were exposed to relive or recount their trauma showed signs of reinforced PTSD. The World Health Organization has found that the victims do best in settings which provide a sense of security and acceptance within the community, followed by some culture-specific cleansing ritual to make the sufferer feel at home again. Since we don’t have these rituals, we need to innovate our own to healing PTSD.

Homeless veterans are facing multiple problems. Military may return home unprepared to cope with PTSD and then to top it off, face financial and communication problems with their loved ones. PAIRS has developed a unique program to help the military and their families develop problem-solving and communication skills to strengthen family life to ensure veterans will not end up alone and homeless. PAIRS has been doing an excellent job to teach veterans and their families how to solve their problems of daily living, however, it does not include therapeutic intervention for post-traumatic stress (PTSD). Nearly 131,000 veterans are homeless on any given night. Most of them are suffering from post-traumatic stress (PTSD). Now let me return to the nearly 131,000 veterans which are homeless on any given night. Nearly twice as many are estimated to be homeless over the course of the year. The total would be over 262,000 veterans homeless. The data was provided by Kelly Kent, Sr. Program Manager of Strategic Partnerships at the Corporation for Supportive Housing.

Veterans account for one third of the men that are homeless. Many of them are chronically homeless.

These are the men that have fallen between the cracks of the system, presumably without the safety net of disability to rely on. These are the veterans that have put their lives on the line in patriotic duty.

At times, unprepared for the responsibilities of civilian life, after deployment, suddenly a dual crisis may set in. One is caused by having to process events which happened overseas, possibly PTSD. The other is caused by feeling stuck and ill-prepared to face the responsibilities of life back home.

Stress and anxiety may set in about financial struggles, mortgage payments, credit card debt and insufficient budgeting skills.

Before deployment deeply in love, couples may experience a change in their relationship as time moves on. The partner who returns from deployment may not be the same anymore. Not each spouse is willing or able to live with a partner suffering from PTSD or physical disability.

Let us assume for a moment that the only issues a young couple faces are budgeting skills, relationship and communication skills. PAIRS has been offering an easy solution.The program of PAIRS is geared to very low income singles and couples. The emphasis is to improve communication skills, problem solving and emotional understanding among couples. 93% of the participants showed sustained improvements in resolving conflicts and overall relationship satisfaction. Looking at the PAIRS program, the mission is clear: Once individuals learn to resolve conflicts in all areas of life, they will also be able to maintain homes, families and relationships.

There are all these disturbing news about veterans committing suicide and veterans’ accidental death due to powerful drug cocktails. When I came across the announcement about  Neuro-Bio blood tests and neuroceuticals for PTSD out of the cutting-edge field of neuroscience it felt like a breath of fresh air and renewed hope for all the sufferers of PTSD, their families and caretakers.

He contributed this fact to the greater calcium built-up in the arteries which hardened the arteries. If the buildup in the arteries leads to the heart,  a heart attack may be the result. If on the other hand, the blockage is in the artery  to the brain, a stroke may occur.

Unfortunately, Dr. Ebrahimi was not in a position to rule out any other cause for the calcium built-up like the Neuro-Bio tests promise to be able to. Exposure therapy has been attempted to help burn victims and PTSD patients desensitize from having been caught in a fire. However, instead of calming and soothing the physical pain, therapists witnessed how the patients were heating up even more so while exposed to the virtual reality of the usual PTSD exposure therapy scenario.

Prof. Hunter Hoffman had the ingenious idea that, if the patients’ burning flesh could be cooled down, patients might experience less discomfort and develop less anxiety and less reinforcement of their trauma while waiting for their dressing to be changed in the treatment room.

Prof. Hoffman and a few others created a unique virtual reality game: a snowball fight which cooled his patients flesh and took their minds off the pain of treatment. Patients put on helmets and noise-canceling headphones and emerged themselves into the Snow World to throw snowballs at snowmen, penguins, igloos and mammoths.

Prof. Hoffman’s idea worked: patients acknowledged that the video game had a cooling effect on their flesh and they felt calmer about treatment.

It is great that burn patients are getting an icy distraction from their physical pain in some trauma centers. Soldier, would you like it if your DNA were to be tested and used in your assessment at all stages of your military career? That’s what a private consulting group named JASON highly recommends to the DoD. They would love to get their hands on this contract to conduct a mass genetic analysis of the military.

It would not only bring good money, but such a mass genetic analysis is only possible in the military, since DNA testing of employees was banned in the private sector. JASON even promised DoD to include certain traits and talents which the military may be looking for. They do admit that their understanding of linking genotypes and phenotypes (genetics with human characteristics and environmental influences) is limited and that such a large data base like the military would present its own challenges. In other words, they are not sure whether they can deliver what they promise.

In 2004 a poll showed that 92% of Americans were against allowing employers access to genetic test results.That survey was conducted by John Hopkins Genetics and Public Policy Center. Furthermore, the Genetic Information Nondiscrimination Act (GINA) was passed by Congress in 2008. It bans employers and health insurance companies from using their employees genetic data. Members of the military were not included.  From then on, GINA has been considered proof that nobody in this country should be discriminated against based on genetic code. It would violate the norms of our society.

One could argue that the military has already been using tiny portions of the genetic code to identify body parts of fallen soldiers and later on, included criminal investigations. Such a tiny portion of the genetic code cannot be compared to analyzing the entire genetic code of members of the military. Presently, the military holds 3.2 million samples for identification purposes.

ACLU suggested that, if the Pentagon would want to research soldiers’ genetic codes, it ought to undertake this research within the same scientific parameters and protocols like the ones civilian scientists are subject to. These protocols would need to guarantee the same meaningful informed consent, institutional review boards and the usually accepted norms of human-subject research, and only then, should it be allowed to carry out this kind of research.

There is no legitimate reason why the military should be allowed to deviate from the rules of society established by Congress.

To analyze limited portions of the genetic code for medical reasons to rule out certain progressive diseases, e.g. Huntingdon’s Disease is useful. To analyze a whole genetic code on a mass scale is a scientist’s dream, even though as we have learned meanwhile, the DNA is not stagnant, but is apt to change just as the brain changes itself.
It implies that interpretations of the whole genetic code, its genotype and its phenotype, might not be able to give the information originally contracted for. There might be dozens of features far too ambitious to be included in any DNA assessment. DNA vulnerability to PTSD belongs to them.

This idea brings me back to wanting to tell you about my “Healing PTSD Trauma” on CD. The result shows that in a war zone non-combatant military is six times more at risk to develop post-traumatic stress (PTSD) than in a  non-war zone. In a war zone, non-combat personnel is exposed to the same life-threatening events, explosions, rockets, etc. as the troops while at the same time, the non-combatants are transporting and treating severely wounded soldiers.They handle body parts and the dead.

It has been hypothesized that combat soldiers have an advantage compared to non-combatant personnel. Combat soldiers are trained to expect life-threatening events and have the chance “to fight back” which may make them less vulnerable to PTSD than non-combatants.

This last point has its merits and needs to be taken into consideration.

Of course, when faced with the symptoms of post-traumatic stress (PTSD), both combat soldiers and non-combatants encounter the same challenge: They need to fight their PTSD. ‘Healing PTSD Trauma‘ provides the unique advantage that it will heal mild or severe symptoms of post-traumatic stress (PTSD).

Before I will tell you about the unique advantages of ‘Healing PTSD Trauma’ to heal your symptoms of post-traumatic stress (PTSD) caused by your war trauma, I want to fill you in on a new research study.

The study which I am referring to was conducted to compare how much more efficiently patients would reduce their symptoms of PTSD, if an anxiety reducing drug was administered before exposure therapy. One patient received the drug, the other patient a placebo.
While I was looking at the test results, questions come to mind.

The case study was published by Rachel Yehuda et al in the European Journal of Psychotraumatology on 12/27/2010. It is entitled “Glucocorticoid augmentation of prolonged exposure therapy: Rationale and Case Report” Rachel Yehuda is affiliated with the Mount Sinai School of Medicine, New York. Glucocorticoid is hydrocortisone, a pain relief drug in the NSAID category (a steroid). 30 mg of hydrocortisone were administered to Patient A 30 min. before his exposure therapy started. Patient B received a Placebo also 30 min. before his session started.

The purpose of this experiment was to establish whether a low dosage of glucocorticoid would reduce treatment length for exposure therapy. Ideally, the drug would calm patients enough so they would more easily revisualize their war trauma.

Anxiety Clinics have been founded in Australia to reduce their patients’ fear. These patients would receive a small amount of D-Cycloserine before the beginning of the treatment sessions[http:anxietyclinic.com.au/D-Cycloserine.html].

According to www.neurotalk.psychcentral.com a typically low dosage for pain relief of hydrocortisone is 5 mg three times per day. In the experiment the PTSD patients received 30 mg of hydrocortisone which is not a mild dosage.

After 10 sessions of receiving 30 mg of Hydrocortisone before each session, Patient A’s PTSD symptoms were 50% lower than at the beginning of the research study. 50% reduction of symptoms is considered a significant difference.

Patient B was given a placebo 30 min. before his sessions. He only reduced his PTSD symptoms by 15.8%. At times, Patient B had refused to relive his trauma, or to execute assignments. He had declared his doubts about the benefits of exposure therapy. As it turned out he had not only avoided to engage at times, however, he had left out sharing a few rather significant details  While he did make progress his desire to avoid exposure therapy altogether and his not having received the hydrocortisone are interpreted as two contributing factors for not reducing his PTSD symptoms to a greater degree.

Comparing the two case stories, it occurred to me, that while both veterans experienced significant war trauma, no mention had been made that apparently only Patient B had killed. His guilt feelings might have been rather deep and not easily solved by regular exposure therapy. He might also not have felt secure enough to bare his soul, especially at the beginning.

Now that we have looked at the shortcomings of exposure therapy I will come back to my ‘Healing PTSD Trauma’.

Tetris To Beat PTSD–Not Yet

So far, Tetris has only been tested in the laboratory. Developing the theory further and testing it in real-life will be an interesting piece of information. If the theory does pin out and reduces symptoms of PTSD flashbacks, the research team imagines Tetris as an adjunct to counseling or other forms of therapy. Would the notion of exposure therapy, the way we understand it now, change? It is premature to even speculate about it.

In the laboratory test healthy volunteers were asked to watch a movie about traumatic injuries, like car accidents while drivers were under the influence of alcohol. Afterwards, one group was asked to play a game of Tetris for ten minutes. The other group played the word-based quiz game Pub Quiz Machine 2008. The control group did not get to play anything. The evaluation showed that the group that played Tetris had a reduced number of flashbacks about the traumatic injuries which they had viewed. The group which had played the word-based quiz game Pub Quiz Machine 2008 had an even higher increase in flashbacks than the inactive control group. Test results also indicated that playing Tetris only showed a beneficial effect if played within a four-hour time span between watching a stressful movie and attempting to solve the Tetris puzzle on the computer.

Laboratory tests do not replace real-life traumatic situations, known to trigger symptoms of PTSD. Would watching the series “Pacific” on HBO trigger PTSD in you? Pretty unlikely, if you are a healthy adult.

The beneficial effect of this “cognitive vaccine”, as it has been called by the researchers and the media, may not work for everybody. Your PTSD flashbacks may merely be replaced by traumatic flashbacks of playing Tetris. Not everybody considers playing puzzle games a fun activity, in which case the quantity of flashbacks would also increase.

The Oxford University research is based on the concept that memory is being formed in two different channels: 1)through perception, and 2) through context. Tetris appears to work much better than the quiz game for the following reason: Tetris displays visual sensory images which compete with some of the traumatic earlier formed images. In fact, they block involuntary recall. (meaning flashbacks, while voluntary recall stays intact.)

The quizz game is based on asking questions and answers. It neglects  to focus on the spatial visual images (the part that involuntary flashbacks are made of) but concentrates on the verbal, contextual part of the experience. The quizz game leaves the flashbacks intact, even increases them  since it causes extra stress on the system.

Yet the day may come where, you, soldier, while deployed, may be asked , after having nearly been blown to pieces:
“Soldier,  would you like to play a game of Tetris for the next ten minutes? It may reduce the likelihood of your developing PTSD flashbacks.”

How would you react?
